[ISC 2011]大力创新,小心求证——ISC 2011主席Lewis B. Morgenstern教授专访
So I pretty much enjoyed a session on how aggressive we should be in caring for patients with intracerebral hemorrhage.
International Circulation: As chair at the International Stroke Conference, what advice would you give doctors who are dealing with stroke, which is the number one killer in China?
Dr Morgenstern: I think that stroke is a very preventable and very treatable condition. I think that my recommendation is that we use science to guide our therapies and I think that there is a lot of scientific information and guidelines written in how best to manage patients. One of the sessions that was really good was a session about new guidelines that had been published this year. I think those are all readily available in the literature and I think that continuing to follow the scientific evidence is the best we can do for patients.
International Circulation: And lastly, low blood pressure is
associated with reduction of hematoma pooling in acute intracerebral hemorrhaging. What do you think is the optimal blood pressure in cases of intracerebral hemorrhaging?
Dr Morgenstern: I think we really do not know what the optimal blood pressure is there in the latest intracerebral hemorrhage guidelines published by the American Heart Association, we give some broad parameters, there is a table of guidelines in which people may look at but it is by consensus opinion and not by data. So there are ongoing trials – the INTERACT trial – which was mostly done in China is an advanced piece of information at this time. There needs to be more studies, if I can say the INTERACT trial is ongoing right now, which will let us know more about the efficacy about blood pressure reactions.

卒中在中国的死因排行中排在首位,因此中国医生需要具备对该病和相关治疗的最完整的认识。但卒中是可防可治的疾病,我们需要用科学来指导治疗,现在有信息和指南都能帮助我们更好地对卒中患者进行管理,本次大会就有一个会议是关于今年出版的新指南,继续遵循科学证据的发展进行卒中治疗能够使我们更好地为患者服务。最近出现了许多新技术,但仍需要经过检验,这些技术是否能真正地改善患者的治疗就是我们在会议上热议的话题之一,但我想答案我们还不得而知。还有几个议题是关于利用核磁共振成像机检测缺血性性卒中的弥散/灌注不匹配的,但对这一技术的价值仍有争议,并有待更多进一步的研究来确定这种方法的可靠性。
降低血压与减少急性脑出血的出血量有关。但是美国心脏病协会(AHA)最新发布的指南虽然列出了脑出血时的血压参考范围,却没有给出最佳血压,而参考范围也是根据专家共识制定而不是根据研究数据提出的。目前还在进行的主要在中国开展的INTERACT试验,将提供更多这方面的信息。将来我们需要更多关于降压效果的研究,以决定脑出血时的最佳降压目标。
